Our nursing service offers home care assessment and intervention packages, specialising in the management of wounds, medication, continence care, and diabetes.

Comprehensive Health Assessment

Clients receiving a Level 3 or Level 4 Home Care Package are required to have an annual Comprehensive Health Assessment to ensure current medical, social and care needs are being met. This assessment helps your nurse and case manager to better identify your care needs and risks, and helps you to make informed decisions about your care options. Additionally, any new clients will receive a comprehensive assessment to establish their baseline healthcare needs.

This assessment price includes:

  • Home visit consultation to check general, physical and mental wellbeing
  • Liaison with the client
  • Liaison with case managers
  • Completion of required medico-legal documentation
  • Travel

Cognitive Screen

Cognitive screens evaluate the way someone thinks, reasons and processes information. Cognitive screens are for people who may be experiencing memory problems, or who are becoming more forgetful over time. Your nurse can complete a cognitive screen (either the Psychogeriatric Assessment Scale [PAS] or the Rowland Universal Dementia Scale [RUDAS]), to obtain a baseline understanding of your cognition, and to determine your eligibility for the Dementia Supplement from the Government.

This assessment price includes:

  • Completion of the assessment
  • Scoring of the assessment
  • Liaison with the client
  • Liaison with case managers
  • Completion required medico-legal documentation

General Clinical Nursing Services

General clinical nursing services are tailored to meet your needs. During this visit, your nurse will discuss your general and mental wellbeing as well as check your blood pressure, blood sugar, temperature, pulse, respiration rate and weight. Depending on findings your nurse may contact your GP or call an ambulance in the case of an emergency.

This service cost includes:

  • Home visit consultation to check general, physical and mental wellbeing
  • Completion of a progress report
  • Liaison with the client
  • Liaison with case managers
  • Completion of required medico-legal documentation
  • Travel

Medication Management

Medications can play an important role in managing health conditions. For assistance managing medication, our nurse can help you or your carer understand what each medication does, the time of day you should take the medication, and how to manage any side effects, or they can assist you take specific medications. This can happen as a one-off appointment or on a routine basis depending on your medication schedule and needs.

This service cost includes:

  • Education of medications taken, including side effects
  • Delivery of medication including tablets, eye drops and injections
  • Carer education and training (if required)
  • Consultation to check general, physical and mental wellbeing
  • Completion of a progress report
  • Liaison with the client
  • Liaison with case managers
  • Completion of required medico-legal documentation
  • Travel

Wound Care

Our Nurses support people who are referred for both short-term and chronic wound management. Your Nurse will prescribe a wound and skin care routine that promotes wound healing and prevents infection. This service price includes the cost of all the dressings and supplies required to take care of your wound. This service cost includes:

  • Assessment, monitoring and review of wound/s
  • Wound dressing (including all supplies, dressing kits and dressings required)
  • Consultation to check general, physical and mental wellbeing
  • Carer education and training (if required)
  • Completion of a progress report
  • Liaison with the client and family as required
  • Liaison with GP, Wound Care Specialist (if involved in care), or Occupational Therapist for pressure care equipment
  • Liaison with case managers
  • Completion of required medico-legal documentation
  • Travel

Diabetes Care

Diabetes requires careful daily management to ensure you achieve good health outcomes. Your nurse can help to educate you about the symptoms and management of diabetes, assess and monitor your blood sugar levels, and administer insulin if required. This service cost includes:

  • Assessment, monitoring and review of blood sugar levels
  • Insulin administration
  • Urine analysis
  • Consultation to check general, physical and mental wellbeing
  • Carer education and training (if required)
  • Completion of a progress report
  • Liaison with the client and family as required
  • Liaison with GP
  • Liaison with case managers
  • Completion of required medico-legal documentation
  • Travel

Duration: 45 Minutes

Palliative Care

Palliative care involves the care of a person who has a progressive condition, or advanced disease. Your nurse can help to optimise your quality of life, and your families through the provision of holistic care which focuses on improving your physical, mental, and spiritual wellbeing. This service involves:

  • Consultation to check general, physical, mental, and spiritual wellbeing
  • Comfort care including positioning, mouth care, skin care
  • Pain management
  • Medication administration and review as required
  • Carer education and training
  • Completion of progress reports
  • Liaison with the client and family as required
  • Liaison with GP, or Palliative Care team, or Occupational Therapists if equipment is required
  • Liaison with case managers
  • Completion of required medico-legal documentation
  • Travel

Duration: 45 Minutes

Continence Care

Your nurse can assist you to manage any continence or bowel care issues. They provide support and education regarding continence products, help you to care for your catheter or stoma, and manage any infections. This service cost includes:

  • Completion of continence assessment
  • Completion catheterisation and catheter changes (including supplies) – if relevant
  • Care for catheter or stoma (including supplies) – if relevant
  • Management of infection – if relevant
  • Assess general, physical and mental wellbeing
  • Free sample of continence pads
  • Client and Carer training
  • Completion of a progress report
  • Liaise with the client and family as required
  • Liaise with GP or Specialist (if involved in care)
  • Liaise with case managers
  • Completion of required medico-legal documentation
  • Travel
