Navigating My Aged Care.

My Aged Care is a government-funded service that connects Seniors with Aged Care Services and supports. We understand navigating My Aged Care can be a challenging task, but it is essential to ensure you receive the care and support that is needed.

3Bridges is assisting individuals over the age of 65 to navigate the My Aged Care system, through support with registration and information. This may involve explaining the different types of services available, providing guidance on how to navigate the website or phone system, and helping with the completion of any necessary paperwork.

 Additionally, ongoing support to ensure that seniors receive the information they require and to address any concerns or issues that may arise with this process.

YOUTH PROGRAMS

Home Care Packages is a responsive care management and a wide range of services including household chores, social activities, personal care and assist with transport, just to name a few. They are designed for those with more complex care needs that go beyond what the Commonwealth Home Support Programme can provide.

Social Connections & Independent Living Support, as people age, everyday tasks can become more difficult. Even though more assistance is needed, there is still a desire to live at home. Our program helps ageing individuals access entry-level support to stay connected socially and live independently and safely at home.  Email: agedcare@3bridges.org.au

Our aim is to create and maintain well-being, promote healthy communities and assist people to build resilience by way of providing registered NDIS services. Email: disabilitysupport@3bridges.org.au

Physiology, and Nursing services. Our allied health professionals can help you manage your physical or mental health by helping you to maintain and build independence in your home environment and community. While, our nurses specialise in the management of wounds, medication, continence care,

Our HMM team work to keep clients living safely and independently in their own homes. The national supplier of Careport, a portable bathroom solution
providing flexible options for people with accessibility requirements. Our team of qualified tradespeople work with occupational therapists to design the most appropriate home modification solution, including minor and major home modifications. Email: HMM@3bridges.org.au

Located at Summer Hill Public School, our goal is to provide high quality Outside School  Hours Care are for school aged children from Kindergarten to Year 6, in a stimulating, comfortable and supportive playbased learning environment enabling families to confidently pursue work, study or other activities. We are a registered training organisation (RTO 41056) with the Australian Skills Quality Authority (ASQA) and approved to deliver vocational education and training (VET) services. Providing training programs not only in the health sector, but expanding our offering into management and hospitality.
