As we get older, many of us need a little extra support to continue living safely and independently at home. My Aged Care provides funding for eligible older Australians to access personalised care and services that help them remain connected to their community and maintain their independence.
At 3Bridges, we work with you to create a personalised support plan that fits your goals, lifestyle and changing needs.
Our experienced team can provide a range of services including:
- Personal care
- Domestic assistance and household tasks
- Nursing care
- Allied health services
- Social support and community participation
- Transport
- Meal preparation
- Home and garden maintenance
- Simple modifications and assistive technology, are typically assessed by an Occupational Therapist or Physiotherapist.

Who is eligible?
You may be eligible for government-funded services at Home if you:
- Are aged 65 years or over (50 years or over for Aboriginal and Torres Strait Islander people), and
- Requires support to continue living safely and independently at home.
- Complete a My Aged Care assessment, which considers:
- Mobility and physical health
- Daily living abilities
- Cognitive function
- Safety risks
- Social supports and overall care needs

Support Levels
Under the new Support at Home program, funding is based on your assessed care needs. Rather than the previous four Home Care Package levels, the program offers eight ongoing funding classifications, allowing support to be more closely matched to your individual circumstances. People with lower care needs receive a smaller funding allocation, while those with more complex needs receive higher levels of support. Additional short-term funding may also be available for restorative care, end-of-life care, assistive technology and home modifications where eligible.
For people with lower-level support needs, assistance may also be available through the Commonwealth Home Support Programme (CHSP). CHSP provides entry-level support services designed to help older Australians remain independent and living safely at home. Services can include domestic assistance, transport, meals, social support, personal care and home maintenance, depending on your assessed needs.
