Disability Support
Everyone deserves the chance to live their life the way they choose.
We help you connect with service providers, access one to one service options, manage your NDIS plan or connect with social groups. Our promise is to listen to what you want and then help you create a fulfilled life, in your own way and in your own time.
As a registered NDIS provider, 3Bridges Community supports you through all the stages of the NDIS process.

Our aim is to create and maintain well-being, promote healthy communities and assist people to build resilience by way of providing the following registered NDIS services:
Personal care
If you need support with personal hygiene and toileting, along with dressing and maintaining personal appearance, our qualified staff can assist with these personal tasks of your daily life.
Occupational therapy
Keeping safety and independence paramount, our OT’s carry out home assessments to assist you with daily activities and feelings of safety through equipment prescription, home modifications and task adaptation.
Social Support Groups & Centre Based Groups
A range of great programs and groups to promote friendships and connections with peers of similar ages and with similar interests. Choose between a range of community and centre-based peer support activities.
Support Coordination
This support will assist you to build capacity and provide you the skills you need to understand, implement and maximise your plan. A Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Home Modifications
Home modifications are changes to the structure, layout or fittings of a participant’s home so they safely access it and move around comfortably in areas they frequently use. Funding for home modifications might include non-portable ramps, modification of bathroom and kitchen areas and fittings, stair lifts, repositioning switches and power outlets, application of slip resistant coatings and activity monitoring solutions.
Group Counselling
Group counselling is a form of therapy where people with similar experiences/issues come together with a professional therapist.
Assistive Equipment for Recreation
The equipment supports a person with a disability to reach their potential at home, in the community and the workplace. It may be included in your NDIS plan. The equipment noted needs to fall under the category of ‘reasonable and necessary’ to assist you to pursue your goals and meet your needs.
Plan Management
Have choice and control over the providers you use, negotiate pricing, make value for money decisions in line with your plan and view your plan on the my place portal to keep track of your budget.
Domestic assistance
This includes assisting you to undertake and/or develop skills to maintain your home environment or performing essential house cleaning activities that you are not able to undertake so you enjoy being at home in a safe, clean environment.
Exercises physiology
Exercise plays a huge role in healthy ageing and it is an effective way of controlling weight, reducing falls and the risk of developing medical conditions in later life. Our Exercise Physiologists tailor their programs according to your individual needs and aim to work with you in a fun and energising manner.
1:1 Social Support/Community Access
This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.
Specialist Support Coordination
This is a higher level of support coordination. This service is for participants whose situations are more complex and who need specialist support. A Specialist Support Coordinator will assist you to manage challenges in your support environment, ensuring consistent delivery of service.
Individual Counselling
Grow self-knowledge, emotional acceptance and growth and the optimal development of personal resources on a one-to-one basis. Get assistance to work towards your personal goals and gain greater insight into your life.
Rental of Equipment
Toilet and bathroom equipment rental. Short-term rental or hire of our CarePort, an accessible, flexible and portable bathroom solution designed to make life more comfortable.
Assistive Products for Household Tasks
Funding for assistive products covers devices that allow individuals to perform tasks they would otherwise be unable to do or increases the ease and safety with which these tasks can be performed.
Arrowsmith Program
Research has shown it is possible to address learning disabilities by identifying and strengthening weak cognitive capacities. The goal of the program is to help students strengthen the weak cognitive capacities underlying their learning dysfunctions and to enable them to become effective, confident and self-directed learners for life.
